It has to be Carroll surely. Not only has he failed to deliver, his signing, contract size and attitude has been of detriment to moving West Ham forward for a number seasons.

Joey Beauchamp ~ We paid a massive transfer fee (at the time) & he started whingeing like a little girl feeling homesick for Oxford (less than 50 miles away)
Had to be unloaded before he even kicked a ball for us
Benni McCartney ~ Came with a big reputation as the top goalscorer we desperately needed
The only thing big about him was his waistline which just got bigger & bigger as he just took the piss out of the club
Keiron Dyer ~ BFS saw Curbishley coming & probably did his best job for Newcastle by flogging him to us for a kings ransom
Set up permanent residence in the sick room
Marco Boogers ~ Dutch headcase
Disappeared to go to live in his caravan in Holland after one game (or half a game since he got sent off for a ludicrous tackle)

Got to be Alex Song.
Came in with a massive reputation started well and faded and then we signed him again and he was total shit.
The most ironic thing about it is that if we had not signed Song for a second spell Leicester would never have won the league because we would of signed Kante.

The Romanians for me. After seeing what they were capable of in the 94 World Cup, both Dumitrescu and Raducioiu were hugely disappointing.
Raducioiu's attitude was what did for him.

John was a very tall, powerfully built striker ex of Arsenal for whom he'd been prolofiic scoring 149 times in 480 odd games over 12 years
He joined us in 76 playing 28 matches scoring 0..... I remember the North Bank song for his was "Give us a goal, Raddy"

As a West Ham fan I have been conditioned to the almost certainty that any we play we sign will be permanently injured, over the hill or plain fucking useless. Then when the odd Ashton, di Canio or Sinclair comes along it’s a nice surprise.
I can almost point to the time when I finally succumbed to this malaise, when we re-signed Frank McAvennie.
The seeds of doubt were sown at FA-Cup QF replay against Norwich, as news filtered through that John Lyall had got his man the three blokes sitting behind me begun to slate McAvennie as if he had raped their wives

They proved to be right, so Frank McAvennie is my most disappointing signing.

Ted MacDougall was a 1 goal in 2 games his whole career, Man U sold him because he was bought by Frank O'Farrall, who was sacked a few weeks after signing him.
Tommy Docherty didn't fancy him, sold him to us.
I thought he'd score bags full but after a dressing room punch up with Billy Bonds, after he slagged him off for not working..
When was only going to be one winner in that fight.
